Job Title:                    Safe Harbor Services Case Manager/Crime Victim Advocate
Supervisor:                Program Director EYS
Status:                        Full-time/Non-exempt (40 hours/week)
Job Summary: The case manager/advocate provides information, referral, safety planning and other supportive services.  This position is responsible for coordinating the provision of trauma-informed Therapeutic Intervention Services for children, youth, and young adults ages 9 through 24 who are crime and/or abuse victims. They will advocate and connect victims of crime to resources that promote recovery and healing, help youth overcome barriers to accessing services, and advocate for system change to increase victims’ safety.  Collaborative work with other agencies and utilizing evidence based best practices while performing the duties of this position is required. This position uses visual and auditory skills approximately 95% of the time.
DUTIES:

  1. Provide intake assessments for clients and identify housing needs and coordinate initial services. Collaborate with housing case managers to establish safe housing for clients, if necessary.
  2. Maintain a caseload of 4-6 Safe Harbor housing clients and 4-6 CVS clients.
  3. Assist in the facilitation of groups for EYS clients.
  4. Collaborate with outside agencies such as: Support Within Reach, the Family Advocacy Center, Beltrami County Children’s Services/Social Services, Northwest Juvenile Training Center, Northwoods Coalition for Family Safety, the local mental health mobile crisis team, Legal Services of Northwest MN, law enforcement, and others.
  5. Provide advocacy and assist youth who have been trafficked/sexually exploited with transitioning to and maintaining safe stable housing. 
  6. Provide crisis intervention, advocacy, transportation, and referrals to victims of trafficking/sexual exploitation. 
  7. Assess survivors’ living situations, and ongoing development of safety plans. 
  8. Direct outreach to local youth through Drop-In coverage as needed. 
  9. Provide support, advocacy, and crisis intervention for victims of crime.
  10. Coordinate provision of services by mental health therapists to clients which are welcoming, culturally inclusive, physically accessible, and appropriate to all victims seeking assistance.
  11. Provide individual case management to meet the individual’s physical health, safety needs of crime victims and to their families/guardians/caregivers as secondary victims.
  12. Manage budget for the Safe Harbor Services and CVS direct assistance.
  13. Maintain accurate and current case files, program records and other documentation.
  14. Collect, compile, and provide data required by funding sources, monthly board reports and complete required reports as needed.  
  15. Works collaboratively with other team members to ensure services are delivered as part of a holistic wraparound services model.
  16. Commit to being an active participant of a team approach, and supportive of shared programming. 
  17. Demonstrate an understanding of the dynamics of human trafficking/sexual exploitation and how they intersect other aspects of survivor’s lives. 
  18. Understand established best practices for working with youth including but not limited to: Trauma Informed Care, Harm Reduction, Victim Focused Services, Positive Youth Development, etc.
  19. Supports Drop-in operations that include safety and needs assessments, brief mental health/trauma assessments, ability to triage multiple need situations, understand the needs of homeless persons or those at risk of homelessness, provide individual guidance, advocacy, referrals, coordination with other community service providers, networking, case consultation and referral to other program staff, case documentation, statistics, and report writing. The number of hours will be assigned by the EYS Program Director.
  20. Performs other duties as assigned. 
 
QUALIFICATIONS:
  1. Prefer a four-year degree in a human service-related field and/or a minimum of two years direct service with youth and families. Must be willing to consider further training and education as needed to maintain a level of competency for this position.
  2. Self-motivated and committed to supporting Evergreens mission, philosophy, programs and services as well as to working collaboratively with other Evergreen team members.
  3. Commitment to working collaboratively with other agencies and service providers to best address the needs of victims of Human Trafficking/Exploitation. 
  4. Demonstrate understanding of and exercise appropriate boundaries with program participants.
  5. Commitment to and use of integrity and ethical, respectful interaction with youth receiving services, staff/coworkers at Evergreen and representatives from other agencies/service providers. 
  6. Strong written and oral communication skills.
  7. Must have reliable transportation, a valid driver’s license and insurance.  Also, must be eligible for coverage under the agency’s auto insurance policy. 
  8. Demonstrate understanding of and commit to maintaining agency, coworker, and client confidentiality.  Will abide by the Data Privacy Act and the Health Information Portability and Accountability Act.
  9. Must not be disqualified by a MN DHS Applicant Background Study, a MN Bureau of Criminal Apprehension Criminal Background Check via the BCA website and must pass a National Sex Offender Registry Check. 
Physical Demands Related to Duties: While performing duties, the employee is required to talk and hear. The employee is often required to sit and use his or her hands and fingers, to handle or feel. The employee is also required to stand, walk, reach with arms and hands, climb or balance, and to stoop or kneel. Visual abilities required by this job include close vision.  This employee must be able to lift and move items up to 35 pounds.
